Credit Application

If you're interested in opening a line of credit with us just print, fill out and sign our credit application. Once it is completed, fax it to the desired store attention Credit Department (the fax numbers are listed on top of the credit application). We'll contact you in up to 7 business days.

Don't forget, the application must be signed to be considered. If you have any questions please contact creditdepartment@lbugroup.com
